Smooth Threads vs. Barbed (Lifting) Threads

PDO (polydioxanone) is a biocompatible, dissolvable suture material used in cardiac surgery for decades. When placed strategically under the skin, PDO threads serve two purposes: immediate mechanical lifting and sustained collagen induction as the threads dissolve over 4–6 months.

Smooth Threads

Placed in a mesh or linear pattern, smooth threads do not lift but trigger a robust collagen response as they dissolve. Best for improving skin texture, firmness, and fine lines in areas like the cheeks, neck, and décolletage. Often combined with barbed threads for comprehensive rejuvenation. Results develop gradually over 3–6 months.

Barbed (Cog) Threads

Feature tiny barbs that mechanically grip and reposition sagging tissue. Produce immediate visible lifting of the mid-face, jowls, brows, and neck. The lift is enhanced as collagen forms around the threads. Used for patients with visible laxity who want structural repositioning, not just collagen stimulation.

What PDO Threads Can Lift

  • Mid-face and cheek descent
  • Jowls and jawline definition
  • Brow lift (lateral brow elevation)
  • Neck laxity and banding
  • Nasolabial fold softening
  • Marionette lines
  • Lower face and lip area
  • Body areas (abdomen, inner arms, thighs) with smooth threads

What to Expect

1

Consultation & Mapping

Your provider evaluates facial anatomy, skin laxity, and determines the thread count and placement pattern. Photos are taken. Realistic outcomes are discussed, PDO threads work best for mild to moderate laxity; significant sagging may require surgical consultation.

2

Numbing (20–30 Min)

Topical anesthetic and local lidocaine injections are used to ensure comfort. You'll feel pressure and movement during placement but should not experience significant pain.

3

Thread Placement (45–60 Min)

Threads are introduced via a fine needle or cannula. Barbed threads are anchored and the tissue is gently repositioned. The entry points are tiny and typically require no sutures.

4

Recovery (2–5 Days)

Mild swelling, bruising, and dimpling is normal and resolves within days. Avoid strenuous exercise, sleeping on your face, and dental procedures for 2 weeks. Visible puckering smooths out as threads settle.

Why PDO Threads Are Growing Rapidly in the Scottsdale Aesthetic Market

Scottsdale has one of the highest per-capita concentrations of medical spas and plastic surgery practices in the United States, and a clientele that is highly educated about the full spectrum of aesthetic options. PDO threads fill a specific gap in that spectrum: they provide meaningful structural lifting for patients who aren't ready for surgery, have already had surgery and want to extend their results, or prefer to avoid general anesthesia. For our patients in Scottsdale, Paradise Valley, and North Scottsdale, PDO threads are frequently combined with neurotoxins, fillers, and microneedling as part of a comprehensive facial rejuvenation plan that addresses volume, movement, texture, and laxity in a single coordinated approach.

PDO Thread Lift FAQs

Am I a good candidate for PDO threads?

Ideal candidates have mild to moderate skin laxity and good skin elasticity. Younger patients (35–55) with early signs of sagging typically see the best results. Patients with significantly loose skin may benefit more from surgical lifting, we'll give you an honest assessment at consultation.

How long do PDO thread results last?

The threads themselves dissolve within 4–6 months. The collagen produced in response to the threads continues to provide support for 12–18 months. Many patients choose to repeat the procedure annually or combine it with complementary treatments to maintain results.

Is there visible scarring after thread placement?

No. Entry points are made with a fine needle or cannula tip and heal without visible scarring in most patients. You may see small marks for a few days after treatment that resolve completely.

Can PDO threads be combined with Botox or fillers?

Yes, and this is our most common approach. Botox relaxes muscles that pull downward, filler restores volume, and threads provide structural repositioning. Together, they address the three main drivers of facial aging: muscle activity, volume loss, and tissue laxity.

What are the risks of PDO thread lifts?

Risks include bruising, swelling, infection, thread visibility or puckering (typically temporary), asymmetry, and (rarely) thread extrusion. Provider experience and proper technique dramatically reduce complication rates. We use only FDA-cleared threads and perform this procedure regularly.
